Fruit intake, exercise, and anaphylaxis

The report on "endurance exercise after orange ingestion cause anaphylaxis" which is food-dependent exercise-induced anaphylaxis (FDEIA), a form of exercise-induced anaphylaxis is very interesting. [1] This kind of anaphylaxis has been recently emerged and widely reported. The complex mechanism of food intake, exercise, and abnormal immune response is the topic for further research. Indeed, FDEIA is uncommon and can be serious but usually underdiagnosed. [2] FDEIA is also believed to be an explanation for unexplained death among sportsmen and athletes. [3] It should be noted that not only fruit but also other kinds of foods including to the gluten can be the triggers for anaphylaxis episode. [2],[3],[4] The problem can be serious and repeatedly occur. In the author's experience, a case of FDEIA presenting with several episodes of anaphylactoid attacks and anaphylaxes after exercises is seen and the final diagnosis has to be done by food allergen study (in this case, the wheat is the causative food allergen). Specific antigen from food detection is specific investigation for final diagnosis. For the case of orange related FDEIA, 3"9 kDa and 53 kDa" are the specific antigens to be tested. [5] Since the condition is severe and can be fatal, prevention is the best suggestion. This condition is the new issue for sports medicine. [3] There are several attempts to find proper method for the prevention of such episode. However, a good simple method is abstinence for postprandial endurance exercise. [2],[3],[4] In any patients with unexplained hypersensitivity problem, the study of food allergen should be considered.
